• My driving lesson will be provided at this address: Bronx, NY 10474
• I am responsible for arriving on time for my lessons/classes.
• I understand that a 24 hour notice is required for cancelling my lessons or the lesson will be lost.
• I understand that a 72 hour notice is required for the cancellation of a Road Test Appointment or the Road Test will be lost and will need to be repurchased.
• I will be prepared for my lessons, read the course materials before class and have my permit and documents with me at all times.
• I will report any license restrictions to DMV and/or my instructor.
• I will provide NEW YORK COMMERCIAL DRIVING SCHOOL with an original validly signed copy of my Medical Authorization.
• I understand that I can be denied a lesson if I show up to class without my permit and the lesson will be forfeited.
• I will arrive on time for my Road Test Appointment with my DOT medical Cad NYS Permit and Driver License.
• I understand that my Road Test will be cancelled if I do not have my NYS Permit and/or my Medical Authorization, eyeglasses (if needed) with me at the time of the examination.
• I understand that NEW YORK COMMERCIAL DRIVING SCHOOL is not responsible for any missing documentation I may not have at the Road Test site, and I will have to reschedule and repay the fee for my Road Test Exam.
• I understand that I cannot bring anyone else to my scheduled lessons or Road Test Exam.
• I understand that any problems or administrative issues, class scheduling or with the driving school should be discussed with management at the school.
• I understand that requesting my instructors cell phone number is against my contractual obligations with NEW YORK COMMERCIAL DRIVING SCHOOL.
• I understand that all lessons are scheduled through the office and that I will receive text reminders for my lessons.
